Titulado: Challenging frontiers: Canine sporotrichosis due to&nbsp;<em>Sporothrix brasiliensis,<\/em>&nbsp;first case in Chile.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>El objetivo de este informe es describir el primer caso de&nbsp;<em>S. brasiliensis<\/em>&nbsp;en un perro dom\u00e9stico en la Regi\u00f3n Metropolitana, Chile. La importancia de este hongo zoon\u00f3tico va en crecimiento y es urgente la implementaci\u00f3n de un enfoque de Una Salud (ONE HEALTH) , que integre esfuerzos de salud humana, animal y ambiental, es esencial para mitigar el impacto de esta enfermedad emergente.<\/strong><\/h4>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p><strong>CASO CL\u00cdNICO<\/strong><\/p>\n\n\n\n<p>Llega a consulta una hembra d\u00e1lmata intacta de 5 a\u00f1os fue remitida para una segunda opini\u00f3n a una cl\u00ednica veterinaria el 15 de febrero de 2023 con antecedentes de un a\u00f1o de evoluci\u00f3n gradual de la piel con un patr\u00f3n ulcerativo que afectaba las extremidades distales y la frente. La paciente no ten\u00eda antecedentes de haber viajado hacia o desde \u00e1reas end\u00e9micas donde se haya descrito previamente\u00a0<em>S. brasiliensis<\/em>, ni antecedentes de haber estado en contacto con gatos dom\u00e9sticos sanos o infectados. La propietaria inform\u00f3 que hab\u00eda sido tratada con altas dosis de prednisona y antibi\u00f3ticos, pero con sin mejor\u00eda.<\/p>\n\n\n\n<p>Al examen f\u00edsico present\u00f3 alopecia cicatricial multifocal, eritema, diversas f\u00edstulas con varios canales de drenaje y lesiones cut\u00e1neas ulcerosas con pus (&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#fig1\">Fig. 1<\/a>&nbsp;). El paciente se encontraba levemente let\u00e1rgico, con signos de dolor moderado y puntuaci\u00f3n de condici\u00f3n corporal de 4\/9. Los ganglios linf\u00e1ticos estaban dentro de los par\u00e1metros normales. En la primera cita se realiz\u00f3 citolog\u00eda con hisopo que mostr\u00f3 inflamaci\u00f3n neutrof\u00edlica, neutr\u00f3filos degenerados, macr\u00f3fagos vacuolados, no se encontraron microorganismos ni queratinocitos nucleados. En esta oportunidad se solicit\u00f3 cultivo bacteriano y antibiograma del corte de las lesiones, adem\u00e1s de hemograma y perfil bioqu\u00edmico. En espera de este resultado, al paciente se le administr\u00f3 una dosis \u00fanica de cefovecina a dosis de 8 mg\/kg (Zoetis, Las Condes, Chile).<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"2370\" height=\"2195\" src=\"https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg\" alt=\"\" class=\"wp-image-23227\" srcset=\"https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 2370w, https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 300w, https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 1024w, https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 768w, https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 1536w, https:\/\/cdn.vetpraxis.net\/wp-content\/uploads\/2024\/05\/Sporothrix-brasiliensis-en-un-perro.-zoonosis.-dermatologia-veterinaria-lesion-dermatologica-perro-piel-pierna-perro-1.jpg 2048w\" sizes=\"auto, (max-width: 2370px) 100vw, 2370px\" \/><\/figure>\n\n\n\n<p>Figura 1&nbsp;.&nbsp;Paciente con lesiones ulceradas, h\u00famedas y sanguinolentas distribuidas en extremidades anteriores, posteriores y cara.<\/p>\n\n\n\n<p id=\"p0045\">El hemograma completo revel\u00f3 anisocitosis leve, poiquilocitosis, policromasia, neutr\u00f3filos degenerados, desviaci\u00f3n izquierda, normoc\u00edtica normocr\u00f3mica y anemia regenerativa; mientras que la bioqu\u00edmica sangu\u00ednea elev\u00f3 la fosfatasa alcalina (500 UI\/L). El cultivo y antibiograma mostraron sensibilidad&nbsp;<em>de Staphylococcus pseudointermedius<\/em>&nbsp;a enrofloxacino; clindamicina; eritromicina y vancomicina. Al perro se le recet\u00f3 prednisona 1 mg\/Kg (Laboratorio Chile, \u00d1u\u00f1oa, Chile) y enrofloxacino 5 mg\/Kg (Rostrum, Drag Pharma, Quilicura, Chile) durante 15 d\u00edas; despu\u00e9s de ese per\u00edodo, no se observaron signos de mejor\u00eda. Se realiz\u00f3 una biopsia incisional de una de las lesiones ulcerosas ubicadas en la extremidad anterior derecha para an\u00e1lisis histopatol\u00f3gico.<\/p>\n\n\n\n<p id=\"p0050\">El estudio histopatol\u00f3gico revel\u00f3 hiperqueratosis, acantosis y ulceraci\u00f3n a nivel epid\u00e9rmico. Mientras que, el tejido d\u00e9rmico y subcut\u00e1neo mostr\u00f3 la presencia de una intensa inflamaci\u00f3n piogranulomatosa, que consisti\u00f3 principalmente en histiocitos epitelioides, c\u00e9lulas gigantes multinucleadas. En el tejido circundante hab\u00eda infiltraci\u00f3n con neutr\u00f3filos y linfocitos. Se detectaron numerosos elementos f\u00fangicos dentro del citoplasma de los histiocitos o, ocasionalmente, de c\u00e9lulas gigantes multinucleadas. Por lo general, se caracterizaban por estructuras parecidas a levaduras, redondas u ovaladas, con una pared celular gruesa y refringente, que a menudo aparec\u00edan como grupos. Los elementos f\u00fangicos fueron positivos a la tinci\u00f3n PAS, lo que indica la presencia de paredes celulares f\u00fangicas ricas en polisac\u00e1ridos. Mientras que el tejido d\u00e9rmico y subcut\u00e1neo mostr\u00f3 la presencia de intensa inflamaci\u00f3n piogranulomatosa nodular con fistulizaci\u00f3n (flecha gruesa). Tinci\u00f3n H\/E 40x.&nbsp;<strong>B.<\/strong>&nbsp;A mayor aumento la inflamaci\u00f3n piogranulomatosa subcut\u00e1nea que consist\u00eda principalmente en histiocitos y linfocitos epitelioides, con presencia de menos neutr\u00f3filos. Se detectaron algunos elementos f\u00fangicos positivos a la tinci\u00f3n PAS dentro del citoplasma de los fagocitos. Tinci\u00f3n PAS 1000x.<\/p>\n\n\n\n<p>Posteriormente, se recomend\u00f3 tomar un nuevo procedimiento de muestreo de las lesiones restantes y enviarlo al Laboratorio de Microbiolog\u00eda Cl\u00ednica y Microbioma, de la Facultad de Medicina Veterinaria de la Universidad Andr\u00e9s Bello. En espera de los resultados del cultivo f\u00fangico, se inici\u00f3 clotrimazol t\u00f3pico al 1% dos veces al d\u00eda (Laboratorio Chile, Santiago, Chile); ba\u00f1os con miconazol 2% + clorhexidina 2% (Regepipel, Drag Pharma, Quilicura Chile) dos veces por semana y terbinafina oral 40 mg\/kg\/una vez al d\u00eda (Laboratorio Chile, Santiago, Chile) [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib12\">12<\/a>&nbsp;], durante cinco semanas y se realiz\u00f3 una nueva revisi\u00f3n. hecho, donde el propietario no vio ninguna mejora.<\/p>\n\n\n\n<p>La identificaci\u00f3n polif\u00e1sica se realiz\u00f3 en una cabina de bioseguridad BSL-2. Inicialmente se realiz\u00f3 un examen microsc\u00f3pico directo de la muestra con tinci\u00f3n Diff Quick, donde se observaron blastoconidias escasas. Luego, la muestra de tejido se pic\u00f3 y se sembr\u00f3 en agar glucosa Sabouraud (SGA) que conten\u00eda cloranfenicol (0,05 g\/l) y cicloheximida (0,4 g\/l) (Merk, Rahway, Nueva Jersey, EE. UU.), y se incub\u00f3 a 25 \u00b0C y 37 \u00b0C durante 10 d\u00edas. Las caracter\u00edsticas macro y micromorfol\u00f3gicas de la colonia permitieron la identificaci\u00f3n presuntiva de&nbsp;<em>Sporothrix<\/em>&nbsp;spp. Mientras tanto, el estudio genot\u00edpico de la colonia aislada incluy\u00f3 la extracci\u00f3n de ADN realizada con el kit Quick-DNA Fungal\/Bacterial (Zimo Research, Irvine, CA, EE. UU.), siguiendo las instrucciones del fabricante. La amplificaci\u00f3n por PCR se realiz\u00f3 utilizando los cebadores universales ITS5, ITS4 y calmodulina. Las secuencias obtenidas fueron editadas, ensambladas y analizadas comparando la similitud de nucle\u00f3tidos utilizando la herramienta BLAST del Centro Nacional de Informaci\u00f3n Biotecnol\u00f3gica (NCBI). Las secuencias de aislados que presentan 100% de identidad gen\u00e9tica con&nbsp;<em>S. brasiliensis<\/em>&nbsp;(c\u00f3digo de acceso OR852420, PP083686).<\/p>\n\n\n\n<p>La concentraci\u00f3n m\u00ednima inhibitoria (CMI) se determin\u00f3 mediante microdiluci\u00f3n en caldo, seg\u00fan el documento M38-A2 del Instituto de Est\u00e1ndares Cl\u00ednicos y de Laboratorio (CLSI) (est\u00e1ndar CLSI M38) [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib13\">13<\/a>&nbsp;]. Los medicamentos probados fueron anfotericina B (AMB), fluconazol (FLC), itraconazol (ITR), ketoconazol (KET), posaconazol (POS), voriconazol (VRC) y terbinafina (TRB) (Sigma-Aldrich, St. Louis, MO, EE.UU). Brevemente,&nbsp;se prepar\u00f3 un in\u00f3culo de cada cepa de 1-5 \u00d7 10&nbsp;<sup>4<\/sup>&nbsp;conidios\/ml en soluci\u00f3n salina est\u00e9ril, incluidas las cepas. Las CMI se determinaron mediante inspecci\u00f3n visual despu\u00e9s de 48 a 72 h de incubaci\u00f3n a 35 \u00b0 C (est\u00e1ndar CLSI M38) [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib13\">13<\/a>&nbsp;]. Los azoles demostraron buena actividad contra las cepas analizadas, mostrando un rango de CMI de 0,25 a 1 \u03bcg\/ml, con excepci\u00f3n del fluconazol que mostr\u00f3 una CIM &gt;64 \u03bcg\/ml. AMB mostr\u00f3 valores de CIM entre 0,5 y 1 \u03bcg\/mL, sin embargo TRB mostr\u00f3 CIM &gt;16 \u03bcg\/mL.<\/p>\n\n\n\n<p><span style=\"font-size: revert; color: rgb(31, 31, 31); font-family: ElsevierGulliver, Georgia, &quot;Times New Roman&quot;, Times, STIXGeneral, &quot;Cambria Math&quot;, &quot;Lucida Sans Unicode&quot;, &quot;Microsoft Sans Serif&quot;, &quot;Segoe UI Symbol&quot;, &quot;Arial Unicode MS&quot;, serif; white-space-collapse: collapse;\">El paciente a\u00fan no respond\u00eda despu\u00e9s de cinco semanas con terbinafina, por lo que se inici\u00f3 con itraconazol (10 mg\/kg\/sid) durante dos semanas. Durante el tratamiento con este nuevo medicamento antif\u00fangico, el due\u00f1o inform\u00f3 que el perro ten\u00eda dolor y dificultad para caminar, por lo que rechaz\u00f3 la terapia y decidi\u00f3 sacrificar al paciente por su mala calidad de vida.<\/span><\/p>\n\n\n\n<p id=\"p0085\">En cuanto a los signos del paciente, estos coincidieron con los informados en la literatura anterior [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib7\">7<\/a>&nbsp;,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib9\">9<\/a>&nbsp;,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib18\">18<\/a>&nbsp;]. Al perro en este informe se le diagnostic\u00f3 una presentaci\u00f3n cut\u00e1nea sin afectaci\u00f3n de los ganglios linf\u00e1ticos y un patr\u00f3n ulcerativo que afectaba las extremidades distales y la frente, lo cual concuerda con G\u00f3mez-Gaviria et al. [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib17\">17<\/a>&nbsp;] hallazgos, donde la forma de enfermedad m\u00e1s frecuente fue la cut\u00e1nea, con el 54,5% de los casos de los perros estudiados, y con Sep\u00falveda Boechat et al. [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib7\">7<\/a>&nbsp;] donde del total de 47 perros, 19 reportaron lesiones cut\u00e1neas y 19 lesiones mucosas y cut\u00e1neas. Mientras que en Barreto Fernandes Figueiredo et al. [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib19\">19<\/a>&nbsp;] informan que las presentaciones m\u00e1s frecuentes fueron la forma mucosa y la forma cut\u00e1nea localizada\/fija. En general, todos los informes mencionados anteriormente describieron ulceraciones en las extremidades y en la frente u otras zonas de la cara. Adem\u00e1s, los par\u00e1metros sangu\u00edneos del paciente mostraron similitudes con informes anteriores, destacando la inflamaci\u00f3n piogranulomatosa [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib20\">20<\/a>&nbsp;] y la anemia regenerativa normoc\u00edtica normocr\u00f3mica [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib18\">18<\/a>&nbsp;].<\/p>\n\n\n\n<p id=\"p0090\">Entre los tratamientos disponibles para la esporotricosis canina, el itraconazol ha sido el m\u00e1s utilizado hasta la fecha, solo o asociado con otros f\u00e1rmacos [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib7\">7<\/a>&nbsp;]. Adem\u00e1s, seg\u00fan Gremi\u00e3o, es un antimic\u00f3tico de primera l\u00ednea para el tratamiento de la esporotricosis humana y animal [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib5\">5<\/a>&nbsp;].<\/p>\n\n\n\n<p id=\"p0095\">Antes de recibir los resultados de la prueba antif\u00fangica, el paciente inici\u00f3 tratamiento con el agente antif\u00fangico terbinafina, pero no se detectaron signos cl\u00ednicos de recuperaci\u00f3n, y posteriormente demostr\u00f3 no tener suficiente&nbsp;actividad&nbsp;<em>\u201cin vitro\u201d<\/em>&nbsp;. Existe evidencia de que la terbinafina se ha utilizado anteriormente para el tratamiento de la esporotricosis canina con resultados exitosos [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib13\">13<\/a>&nbsp;], sin embargo, no es el antif\u00fangico de primera elecci\u00f3n. Luego se prescribi\u00f3 itraconazol durante dos semanas hasta la reevaluaci\u00f3n cl\u00ednica. Sin embargo, el propietario opt\u00f3 por la eutanasia en otro centro veterinario.<\/p>\n\n\n\n<p id=\"p0100\">Los autores destacan la importancia de un diagn\u00f3stico r\u00e1pido, despu\u00e9s del cual se pueden tomar decisiones adecuadas sobre el mejor tratamiento individual para cada paciente junto con un seguimiento adecuado [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib5\">5<\/a>&nbsp;]. Se recomienda realizar los estudios pertinentes en caso de sospecha de esporotricosis para prevenir la propagaci\u00f3n de la enfermedad en caso de confirmaci\u00f3n. Adem\u00e1s, se recomienda encarecidamente seguir las recomendaciones de tenencia responsable de mascotas y animales de compa\u00f1\u00eda. Como resultado, se reduce el contacto entre animales dom\u00e9sticos y salvajes y por tanto la posibilidad de transmitir el hongo a la vida silvestre. Esto pone de relieve la importancia de abordar el problema desde la perspectiva de Una Salud [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib7\">7<\/a>&nbsp;].<\/p>\n\n\n\n<p id=\"p0105\">Considerando la r\u00e1pida expansi\u00f3n de&nbsp;<em>S. brasiliensis<\/em>&nbsp;en Brasil durante la \u00faltima d\u00e9cada [&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S221175392400006X#bib5\">5<\/a>&nbsp;], los autores enfatizan la necesidad de tomar medidas no solo para poder detectar y confirmar la infecci\u00f3n sino tambi\u00e9n proponer estrategias para prevenir y controlar este hongo en Chile. , pa\u00eds que apenas comienza a observar los primeros casos de esporotricosis por&nbsp;<em>S. brasiliensis<\/em>&nbsp;.
